Okay so I was going through the medical exam part of a term life insurance application last month, the kind where a nurse comes to the house and takes measurements, and the whole underwriting conversation kept circling back to one number on the form. BMI. I know the number, I'd seen it on a doctor's chart before, but I'd never actually looked into where it comes from or what it's supposed to mean. Turns out it's just a formula anyone can run themselves, it's not some mystery box a doctor calculates. So here's what I found once I actually dug into it.

The official category cutoffs

This is the scale the CDC and WHO use for adults, and it's the same one that shows up on insurance paperwork and doctor's office printouts. Five bands, same formula the whole way across.

How to actually run the number

You don't need the insurance nurse or a doctor's office scale for this. It's three inputs and one calculation, and once you've done it once you'll never forget it.

Get your height and weight
Metric works cleanest: height in meters, weight in kilograms.

Square the height, divide the weight by it
Weight (kg) ÷ height (m)². In pounds and inches, it's 703 × weight ÷ height².

Match the result to a category
Compare against the four ranges above. That's the whole calculation.
03 · The limits

What BMI doesn't tell you

The formula was built in the 1830s by a Belgian mathematician looking at population averages, not individual health. It's a screening tool, not a diagnosis, and it has real blind spots worth knowing before you take the number too seriously.

Muscle vs. fat

The formula can't tell the difference. A muscular athlete and someone carrying extra fat at the same height and weight get the exact same score.

Where the weight sits

Fat stored around the waist carries more cardiovascular risk than fat elsewhere, and BMI has no way to see that. Waist circumference catches what BMI misses here.

Different bodies, different baselines

WHO itself recommends lower overweight cutoffs for some Asian populations, and the standard adult ranges don't apply to kids, pregnant women, or older adults losing muscle mass.

Once I understood what the formula was actually built for, most of the confusion around it made a lot more sense.

Treating it as a diagnosis
It's a screening number. A doctor pairs it with blood work, blood pressure, and history before anything gets diagnosed.

Ignoring waist circumference entirely
Two people with the same BMI can carry very different cardiovascular risk depending on where the weight sits.

Applying adult ranges to kids
Kids and teens use age and sex-specific percentile charts, not the flat adult cutoffs.

Comparing across very different builds
A powerlifter and a sedentary person can land on the same number for completely different reasons.

Obsessing over the decimal
A BMI of 24.8 versus 25.1 is not a meaningful health difference on its own, it's a threshold crossing on a population-level tool.
